The main objective of the role"

  •  Carry out all social-related activities, to ease the patient’s situation, maximize his/her adherence to treatment, and improve his/her psychosocial condition.

Responsibilities:

  • Identify the social needs of patients through a social assessment. Respond to the patient’s social needs, according to the scope of the project, by giving direct social support (as transport, food, legal advice, …) and/or referring to other social actors, to actively contribute to the improvement of their quality of living.
  • Contact patients to follow up on the evolution of their condition when needed (home visits, defaulter tracing).
  • Participate in the creation of any material needed for the activities of social workers. Take care of social services facilities, materials, and equipment to ensure continuity.
  • Mapping and regular updates of social partners, local authorities, other NGOs, and governmental social services to improve social services scope and responsiveness.
  • Keep records, statistics, and files updated, reporting any social activity, to prioritize and develop appropriate programs or projects.
  • Perform consultation and motivational interviews for patients as part of a multidisciplinary team.
  • Participate in the activities of harm reduction-related education and awareness both in the community space of the MSF clinic and outreach, according to project objectives.
  • Possibly solving social-related barriers to adherence to the care plan, according to the objectives of the project.
  • Prepare and follow individual plans for specific complicated patients in agreement with the direct manager, reporting and updating step by step the evolution of the situation to other colleagues involved in the management of the case.
  • Represent the social component of the project in team meetings and case management meetings.
  • Actively participate in social team meetings, Hepatitis C meetings, psychosocial meetings, and general meetings.
  • Participate in mobile clinic and outreach activities according to project objectives and evolution.
  • Ensure professional confidentiality is respected.
  • Contribute to quality of care in the clinic.
  • Know, respect, and implement MSF principles (charter), rules, and regulations in daily activities.
  • Maintain an effective and respectful communication with colleagues and supervisor.

Specific Requirements for the Role:

  • Education: a desirable degree in social work.
  • Experience: experience in a similar social job (at least 2 years) is essential. Experience with MSF or other NGOs is desirable.
  • Languages: Farsi and English.
  • Knowledge: essential computer literacy (Word, Excel, and the Internet).
